Privacy policy

This page explains what personal information we collect, why we collect it and how we look after it.

Last updated: 27 October 2026

Who we are

We are Affordable Care Solution Pty Ltd trading as Bridgewell Connect, ABN 32 673 894 481.

Our office is at Unit 4, 195-196 Fullarton Road, Dulwich SA 5065.

We follow Australia's privacy law, the Privacy Act 1988 (Cth), including the Australian Privacy Principles.

What we collect

We collect information you give us through our website forms, by phone, by email or in person.

With your consent, we may also get information about you from other people. This can be the person who referred you, your family, your plan manager, the National Disability Insurance Agency (NDIA) or your other providers.

This can include your contact details, your NDIS details and your support needs.

If you make a referral for someone else, we also collect your own contact details.

Some of this information, such as details about your health or disability, is sensitive. We only collect it with your consent. We take extra care with it.

How we use it

We use your information to:

  • respond to you
  • provide and coordinate your supports
  • claim payment for our services
  • meet our legal duties as a registered NDIS provider

How our website forms work

When you send a form on our website, it is sent by email to our Outlook inbox through Microsoft 365. Your form is not stored on the website.

Our forms use Cloudflare Turnstile to stop spam. Turnstile uses your IP address (the number that identifies your internet connection) and information from your browser to check that a person, not a computer program, is sending the form.

Microsoft and Cloudflare may handle this information on computers outside Australia, such as in the United States.

Who we share it with

We share your information with your consent. For example, we may share it with your providers or your plan manager.

We only share it without your consent when the law requires or allows us to. For example, we may need to keep someone safe from serious harm, or report to the NDIS Quality and Safeguards Commission.

Keeping it safe

We take reasonable steps to protect your information from loss, misuse and people who should not see it.

When we no longer need your information, we delete it or destroy it securely, unless the law says we must keep it.

Your rights

You can ask to see the information we hold about you. You can also ask us to correct it if it is wrong. It is free to ask.

To ask, call us on 0450 879 346 or email info@bridgewellconnect.com.au. We may need to check who you are first, to keep your information safe.

When it is practical, you do not have to give your name, or you can use a different name. We may not be able to coordinate your supports without your details.

If you have a concern about your privacy

If you are worried about how we have handled your information, please tell us. Call us on 0450 879 346 or email info@bridgewellconnect.com.au.

We will look into it and tell you what we find.

If you are not satisfied with our answer, you can contact the Office of the Australian Information Commissioner. Call 1300 363 992 or visit oaic.gov.au.
