Our services

One team to coordinate your supports, so they work together.

What is support coordination?

Support coordination can be funded in your NDIS plan. A support coordinator helps you understand your plan, find the right providers and make your supports work together.

We do the phone calls, the paperwork and the follow-ups, and we build your confidence to manage more of it yourself over time.

Support Coordination

We help you understand your plan, find providers and use your funding well.

  • We explain your plan in plain English, including what each budget can be used for.
  • We find providers that suit your goals, set up service agreements and check that supports start on time.
  • We help you build the skills and confidence to manage more of your plan yourself.
  • We help you prepare for plan reviews and for changes in your situation.

Specialist Support Coordination

More support when your situation is complex, for example with housing, health or the justice system.

  • For participants whose plans fund Specialist Support Coordination, usually because of high or complex needs.
  • We work with health, housing, education and justice services, and help sort out problems between them.
  • We help put together a support plan that keeps you safe and moving towards your goals.
